Join us in a virtual meeting to chat with your heart failure team nurses. Please feel free to discuss how you are managing at home or at work in this this time of social distancing. This group is open to all heart failure patients and their families. You will learn about managing and living well with heart failure in a supportive group environment.

We meet the last Wednesday of every month.
To join from your computer, upon registering, you will receive emailed instructions on how to virtually connect. Patients can access remotely and join from any video-enabled device (smart phone, iPad, laptop, or webcam/desktop).
