DESCRIPTION:***Space is limited for this event\, and registration is required. \nThis free workshop will help you learn about and complete a Nevada Advance Health Care Directive (also called a Durable Power of Attorney for Healthcare) and understand what a Provider Order for Life-Sustaining Treatment (POLST) form is. All necessary forms will be provided.  \nWhat to expect: \n\nLearn the purpose and importance of Advance Health Care Directives. \nReceive a step-by-step review of the Advance Health Care Directive to help you make informed health care decisions. \nLearn about POLST forms and who may benefit from having one. \nAsk questions and receive guidance from Renown staff. \nRenown staff can serve as witnesses and upload completed Advance Health Care Directives to your Renown medical record and the Nevada Lock Box Registry.\n\nPLEASE NOTE: Notary services will not be available\, and copies of completed documents will not be provided. A list of notary services will be provided. \n➡ Please bring a photo ID\, the names\, addresses\, and phone numbers of the individuals you would like to designate as your healthcare decision-makers.  \nDisclaimer:\nTo register online\, you must enter an email address.
