Senior Care Plus Advanced Care Planning Seminar

This workshop is focused on understanding and completing a Nevada Durable Power of Attorney of Healthcare, also known as an Advance Directive, and understanding what a Provider Order for Life-Sustaining Treatment (POLST) is. This workshop is free of charge for SCP members. All required paperwork will be provided by our staff.

What to expect:

• A brief presentation about the document, its contents, and purpose.
• Time for questions.
• Two witnesses will be available to sign to validate advanced directive
• Staff will be on hand to assist with document completion.
• Participants may have their document uploaded into their Renown electronic medical record and/or submitted to the Nevada Living Will Lockbox on their behalf.
